DISTRICT NURSING

The District Nursing Team provides nursing care to the housebound. Our referral criterion includes the following: -

• In-depth assessment of health needs
• Wound and leg ulcer assessment and treatment
• Palliative care / Continuing care
• Clinical procedures such as Hickman/Picc lines, breast drains, pleurex drains,
• Blood tests
• Catheterisations and follow up care
• Carers’ support and advice
• Acute disease management e.g. post-operation care, urinary infection
• Continence Assessment, including bladder and bowel care
• Skin flap injuries or minor injuries
• Vaccinations
• Liaison and multi-agency referrals
• Health Promotion

The District Nursing Team is based within Wyndham House Surgery and consists of:
